How to get a replacement carte Vitale
The replacement request is made online at ameli.fr (section "Ma carte Vitale"), by letter to your CPAM, or in person at a CPAM office. Attach a photocopy of your ID document, a recent proof of address, and if it was stolen, the police report receipt. The replacement is entirely free. Your new card arrives by post within 3 to 4 weeks.
Did you recently change address?
Before making your replacement request, check that your address is up to date on ameli.fr. If your old address is still registered, your new card will go to your former neighbour. Updating your address takes just a few clicks online and takes effect immediately.
Preventing future losses
Don't keep your carte Vitale in your wallet if you don't use it regularly. Keep it in a safe place at home and only take it out on appointment days.
